A new initiative at Campbellford Memorial Hospital (CMH) aims to help patients manage their health at home and reduce unnecessary return visits to the emergency department.
Launched in partnership with health services provider Gotcare, the Rural Stop Gap (RSG) Program supports patients with a blend of home visits and virtual care. The program offers services like remote monitoring, falls prevention, wellness check-ins, and personalized care plans. It also uses artificial intelligence and predictive tools to flag potential issues before they become emergencies.
“The Rural Stop Gap Program is an innovative digital approach to augment our patient care model,” said Heather Campbell, vice-president of patient care and chief nursing executive at CMH. “By providing structured support at home, we can improve patient outcomes, reduce hospital congestion, and enhance overall healthcare access for our community.”
Initially rolled out in the Quinte area for patients in Hastings and Prince Edward counties, the program is now open to all patients receiving care at CMH, regardless of where they live.
Gotcare co-founder and CEO Chenny Xia said the goal is to “bridge the gap between hospital and home care” and ensure patients feel supported while managing their health independently.
The RSG Program supports CMH’s soon-to-be-released strategic plan, which aims to improve healthcare access and innovation in rural communities.
